Application for Graduation
A student is responsible for submitting an official application for graduation and for fulfilling all requirements for the degree in accordance with the regulations of that program, as outlined in this catalog. Students who meet all graduation requirements in a given term but fail to apply for graduation and/or pay their graduation fee until after the semester ends will be graduated at the end of the semester in which their application is received in the office of graduate studies and research, not the term they completed their coursework.

Students must meet all graduation requirements by the official end of the semester in which they have applied to graduate. Failure to do so (Incomplete grades in any course or "X" grades in required courses, no application, etc.) will result in the updating of the student's graduation date to the end of the term /year the work is eventually completed.

The official application for graduation form may be obtained from the dean of graduate studies and research. The application, with required signatures of the advisor and graduate coordinator, must reach the dean of graduate studies and research by November 1 for December graduation and April 1 for May graduation. If the first falls on Sunday, the headline will be the following business day. The diploma fee must accompany the application.

Graduation Fee..........................................$20.00
This fee is paid by each candidate for a degree, to cover the cost of the diploma, leatherette presentation case, reception and other services related to commencement exercises. Not transferable or updateable.

Transcript Fee.............................................$3.00 Same-day service is $10.00 per transcript. Fee charged for the preparation and mailing of each transcript. Every student completing undergraduate or graduate study receives a complimentary copy of the transcript, with each subsequent transcript charged at the established rate. Students requesting that documents be faxed will be assessed a fee of $5.00 ($12.00 for "same day" service).

Thesis
Final copies of theses must be submitted in photo-ready typed format, using a letter quality typewriter or printer. Three bound copies of the thesis are minimal-two for the library and one for the department. Custom frequently dictates two additional copies—one for the thesis advisor and one for the student. The fees for binding and copying must be paid at the time an application for graduation is filed. The official style manual for theses is available from the graduate dean. An approval code from the Institutional Review Board for the Protection of Human subjects is required before data collection involving human subjects may begin. Research protocol guidelines may be obtained from the chair of the IRB-PHS.

Any student who does not register during four consecutive regular semesters will become inactive and will require readmission should the student decide to continue working on a graduate degree. During the inactive period, the statute of limitations will continue to be applied.

This request must be supported by the graduate coordinator before submission to the dean of graduate studies and research.
